Prostate and urology care at GenesisCare Windsor
GenesisCare Windsor is a specialist cancer and diagnostic centre at 69 Alma Road, Windsor.
Its UrologyHub provides access to prostate and urological investigations, while the wider centre provides specialist oncology and radiotherapy services. Current facilities include prostate multiparametric MRI, transperineal prostate biopsy and flexible cystoscopy, alongside other diagnostic imaging and cancer treatments.
This makes Windsor particularly useful for men who need investigation of a raised PSA or suspected prostate cancer, as well as those who have already received a diagnosis and need to consider the different treatment options.
My role is to guide you through that process, interpret the results in the context of your individual circumstances and help you decide what should happen next.
Raised PSA assessment in Windsor
A raised PSA is one of the most common reasons men are referred to me.
Importantly, a raised PSA does not mean that you have prostate cancer.
PSA can also rise because of benign enlargement of the prostate, inflammation, infection, urinary retention and other factors.
If your PSA has been found to be raised or is increasing over time, I will review the result alongside factors such as:
- your age
- previous PSA results
- family history of prostate cancer
- urinary symptoms
- prostate examination where appropriate
- prostate size
- previous MRI or biopsy results
- your overall health
The aim is to assess your individual risk rather than make decisions based on a single PSA number.
Where further investigation is required, this may include a multiparametric MRI scan of the prostate.
GenesisCare Windsor provides prostate mpMRI as part of its specialist diagnostic pathway.
You can read more about what an abnormal PSA means and how it is investigated on my Raised PSA Clinic page.
Prostate MRI and biopsy
Modern prostate cancer assessment usually uses MRI to help determine whether a biopsy is required.
Not every man with a raised PSA needs a biopsy.
If your MRI and other findings are reassuring, it may sometimes be reasonable to monitor the PSA rather than proceed immediately to tissue sampling.
If a biopsy is required, GenesisCare Windsor provides MRI-targeted transperineal prostate biopsy through its UrologyHub.
A transperineal biopsy takes samples through the skin between the scrotum and anus rather than through the rectum. MRI information can be used to target suspicious areas of the prostate while additional samples are taken where appropriate.
The biopsy allows a pathologist to determine whether prostate cancer is present and, if so, its grade.
I then review the pathology alongside the MRI, PSA and other clinical information to determine the significance of the findings and discuss what should happen next.
You can read more about the procedure in my Prostate Biopsy Guide.
If prostate cancer is diagnosed
A diagnosis of prostate cancer does not automatically mean that you need immediate treatment.
Prostate cancers vary considerably in their behaviour.
Some low-risk cancers can be managed safely with active surveillance, while others are better treated with surgery, radiotherapy or another form of treatment.
The right recommendation depends on several factors, including:
- the grade of the cancer
- how much cancer was found on biopsy
- MRI appearances
- PSA and PSA density
- whether the cancer appears confined to the prostate
- your age and general health
- urinary function
- sexual function
- prostate anatomy
- previous surgery
- your own priorities and preferences
This is one reason I believe treatment decisions should be made only after the complete clinical picture has been reviewed.
Multidisciplinary prostate cancer care
All newly diagnosed urological cancer cases under my care are discussed through an appropriate multidisciplinary team meeting, or MDT.
The MDT brings together specialists involved in cancer diagnosis and treatment, which may include urologists, radiologists, pathologists, oncologists and specialist nurses.
The team can review the relevant imaging, pathology and clinical findings and consider the appropriate treatment options.
I then discuss the MDT recommendations with you personally.
An MDT recommendation does not replace an individual consultation. Different treatments can have different implications for urinary function, sexual function, recovery and quality of life, and your own preferences remain an important part of the decision.
The purpose of the MDT is therefore to ensure that your cancer has been considered from several specialist perspectives before you make a treatment decision.
Choosing between surgery, radiotherapy and active surveillance
One of the advantages of seeing patients with prostate cancer at GenesisCare Windsor is the ability to work closely with specialist cancer and radiotherapy teams.
GenesisCare Windsor provides radiotherapy and oncology services alongside its prostate diagnostic facilities.
For suitable patients, the main options may include:
Active surveillance
Some prostate cancers have a low risk of causing harm and can be monitored rather than treated immediately.
Surveillance typically involves a combination of PSA testing, clinical review, repeat MRI and, where necessary, further biopsy.
Radical prostatectomy
Radical prostatectomy removes the prostate and seminal vesicles with the aim of curing prostate cancer that is confined to the prostate or immediate surrounding tissues.
I have performed more than 1,000 radical prostatectomies during my surgical career and have experience in both robotic-assisted and open prostate cancer surgery.
If surgery is the preferred treatment following your assessment and MDT review, I can arrange this within my wider private practice at an appropriate surgical hospital while remaining responsible for your urological care.
You can read more about recovery after surgery in my detailed Robotic Prostatectomy Guide.
Radiotherapy
Radiotherapy is an effective curative treatment for many men with localised and locally advanced prostate cancer.
Because GenesisCare Windsor provides specialist radiotherapy services on site, I can work closely with the treating oncology team where radiotherapy is the preferred option.
In selected patients, additional procedures may be used as part of radiotherapy planning to reduce radiation exposure to surrounding structures.
My aim is not to favour surgery simply because I am a surgeon. The appropriate treatment should be determined by the characteristics of the cancer and the individual patient.
Prostate cancer second opinions
I regularly see men who have already been diagnosed with prostate cancer and would like a second opinion before making a treatment decision.
You may have been advised to:
- start active surveillance
- undergo robotic prostatectomy
- consider radiotherapy
- begin hormone treatment
- have further investigations
- choose between several apparently reasonable options
A second opinion can be especially useful when more than one treatment is possible.
During the consultation I can review your:
- PSA history
- prostate MRI
- prostate biopsy and pathology report
- cancer Grade Group
- staging investigations
- prostate size and anatomy
- urinary symptoms
- sexual function
- previous surgery
- general health
We can then discuss the advantages and disadvantages of the most appropriate treatment options for your situation.
If further specialist input from an oncologist would help before you decide, we can also incorporate it into the multidisciplinary pathway.
What if surgery is the best option?
GenesisCare Windsor is an important part of my consultation, diagnostic and multidisciplinary prostate cancer practice, but the most appropriate treatment is not determined by the clinic where you were initially assessed.
If, after investigation and MDT discussion, radical prostatectomy is the preferred treatment, I can arrange surgery at one of the hospitals within my private practice where the appropriate surgical facilities are available. I currently perform robotic-assisted radical prostatectomy at The Clementine Churchill Hospital, Harrow and The Princess Grace Hospital, London
You remain under my care through the process.
This means you can have your initial assessment and cancer discussions close to home in Berkshire without your treatment options being restricted by the facilities available at a single site.
Similarly, if radiotherapy is more appropriate, I can work with the specialist oncology team to coordinate that treatment.
Other urological conditions I see in Windsor
Although prostate cancer forms an important part of my GenesisCare practice, I also see patients with other urological problems.
These include:
- blood in the urine (haematuria)
- lower urinary tract symptoms
- benign enlargement of the prostate
- recurrent urinary tract infections
- bladder problems
- abnormal urinary imaging
- suspected bladder or kidney cancer
- other general urological conditions
GenesisCare Windsor provides a range of urological diagnostic services including flexible cystoscopy, bladder assessment, ultrasound, urine testing and urinary flow assessment through its UrologyHub.
Where an investigation or operation is better performed elsewhere, I can arrange this at the appropriate hospital while maintaining continuity of your care.
My experience
I have practised as a Consultant Urological Surgeon since 2005.
My specialist interests are in uro-oncology, prostate cancer surgery, laparoscopic surgery and robotic-assisted surgery.
I have performed more than 1,000 radical prostatectomies and more than 600 laparoscopic kidney operations during my surgical career.
I completed my specialist urological training in Oxford and then undertook fellowships at the Cleveland Clinic in the United States and in Nantes, France, further developing my expertise in laparoscopic and minimally invasive urology.
You can find more information about my training and clinical experience on my About Mr Neil Haldar page.

Frequently asked questions
Does Mr Neil Haldar see private patients at GenesisCare Windsor?
Yes.
GenesisCare Windsor is one of my main private clinic locations in Berkshire. I see patients there with raised PSA, suspected or diagnosed prostate cancer and other urological problems.
Can I see you at GenesisCare because my PSA is raised?
Yes.
A raised or rising PSA is one of the most common reasons men arrange an appointment with me.
I can review your PSA results and individual prostate cancer risk and arrange further investigations where necessary.
Can I have a prostate MRI at GenesisCare Windsor?
Yes.
GenesisCare Windsor provides multiparametric MRI of the prostate as part of its specialist prostate diagnostic service.
Whether an MRI is required depends upon your individual circumstances and any investigations you have already undergone.
Can a prostate biopsy be performed at GenesisCare Windsor?
Yes.
GenesisCare Windsor provides MRI-targeted transperineal prostate biopsy through its specialist UrologyHub.
A biopsy is not automatically necessary for every raised PSA. The decision should take account of the MRI, PSA density, examination findings, previous PSA results and individual risk.
Will my prostate cancer be discussed at an MDT?
Yes.
Newly diagnosed prostate cancer cases under my care are reviewed through an appropriate multidisciplinary team meeting.
This allows specialists including urologists, radiologists, pathologists and oncologists to consider the findings and treatment options.
I then discuss the MDT recommendations and the available choices with you.
Can I see you for a prostate cancer second opinion?
Yes.
I regularly see men who have already been diagnosed with prostate cancer and want another opinion before deciding between active surveillance, surgery, radiotherapy or another treatment.
It is helpful to have your PSA results, MRI images and report, biopsy pathology and any staging scans available for review.
Do you perform robotic prostatectomy at GenesisCare Windsor?
If surgery is the preferred treatment, I arrange prostate cancer surgery at an appropriate hospital within my wider private practice where the necessary surgical facilities are available.
Your assessment and follow-up can still take place with me in Windsor where appropriate.
What happens if radiotherapy is more appropriate than surgery?
GenesisCare Windsor provides specialist oncology and radiotherapy services.
If radiotherapy is the most appropriate option, I can work with the oncology team so that your urological and cancer care are coordinated.
Do I need a GP referral?
Not necessarily if you are paying for your own consultation.
If you are using private medical insurance, your insurer may require a GP referral or prior authorisation.
Please check with your insurer before your appointment.
